A. HEALTHCARE PRACTICE BUSINESS FORMATION SERVICES
NOTE: Pricing does not include any filing fees or application fees unless otherwise stated
$2000 - Single Member PLLC Formation Package (New York State)
Includes:
1) Preparation of the business's Articles of Organization (AOO);
2) Filing AOO with the NYSED Office of the Professions and Obtaining a Certificate of Authority;
3) Filing AOO with the NYSDoS Division of Corporations;
4) Filing Certified AOO back to NYSED-OP;
5) Obtaining IRS Employer Identification Number (EIN)
NOTE: This does not include the Publication Services. Section 206 of New York LLC law requires that LLCs/PLLCs publish a Notice of LLC/PLLC Formation once a week for six weeks in two newspapers (one daily and one weekly) that operate from the same county as the PLLC's principal address and file the New York Certificate of Publication for Domestic Limited Liability Company with the New York Department of State within 120 days of formation.

NOTE: A PC is faster to create and more affordable, but it is limited to the practice of only one profession. A PLLC, on the other hand, can practice more than one profession as long as one of its members is licensed In that profession. For example, Random Physical Therapy, PC can only provide PT and cannot add an OT or SLP to the PC practice, while Random Physical Therapy, PLLC can add an OT and/or an SLP owner and be able to provide PT, OT and/or SLP services.

Flat fee billing structure. Once we have taken a look at the document, we will offer a fixed fee to review the contract based on the complexity and length of the contract. On average, a simple 10-page contract that only requires straightforward review and analysis is approximately $350-$400. Rest assured, we will provide you with the cost before the service is provided.
